A New Program Offering  

“Leadership Soil Warming; the Foundation for a Sustainable
Person-Centered Care Journey”

The purpose of the WNYAPCC initiative is to assist facilities and elder care organizations to learn about  person-centered care philosophies, evaluate the current practices in our region, and develop a framework for a sustainable community-wide person-centered care, culture change movement. We have secured subject matter expert Susan Berta, RN, LNHA, Eden Educator, Mentor and Culture Change Guide to lead this essential all day training for any organization embarking upon or currently on their culture change journey.

Leadership Soil Warming; the Foundation for a Sustainable Person-Centered Care Journey:
Is a one-day skill building leadership development program that covers: the essential building blocks for a strong leadership foundation that will drive and sustain person-centered growth and initiatives. Leadership drives all culture and the importance of solidarity and a “oneness” of spirit are crucial to create growth and development in any organization.

Participants will have an opportunity to develop the skills necessary to return to their organizations and develop a leadership pathway that will drive and cultivate the necessary attitudes, behaviors and approaches using soil warming guidelines that will build a strong foundation from which to grow their culture change journey. No matter where your organization is at on their journey, you won’t want to miss this informative session; you will leave with new energy, insight and passion.

Length of Program:
8:30 AM – 4:30 PM (lunch is included)

Program Objectives:

  • Identify core team leaders and determine the “temperature” of the soil within this group
  • Identify areas of personal growth and implement strategies for positive change within the leadership group
  • Provide a method to identify strengths of the core team, celebrate and role model those strengths to inspire growth across the organization
  • Provide a method to identify “opportunities to grow” in a non-threatening manner allowing each team member a voice in order to establish workable solutions growing the individual and the team as a whole
  • To achieve and maintain a higher level of trust and respect among core leadership
  • To create a “oneness of spirit” that role models, drives and sustains person-centered care throughout the organization

Target Audience:
For leaders (defined as those who have direct reports) at every level including household leaders, supervisors, managers, directors and administrators.
